Advanced care planning is a critical piece in healthcare that is often looked at only when a crisis is near or actively occurring. An advance directive and healthcare proxy are the two common forms people can complete in order to assist them in their future healthcare plans. With the current healthcare crisis happening worldwide, it is critical that clinicians have conversations with clients about these documents and the benefits of completing them.
